By booking for class via this website, I acknowledge the following: ​

I understand that by registering a child for class, I am responsible for the child's participation in exercise programmes, classes or events and any related activity conducted by The Slink Academy. The instructions and advice presented by the instructor are designed for those who are in good health. If in any doubt, please consult a doctor before enrolling the child in this or any other exercise programme. The instructor will not be liable for any injuries or damage arising out of participation or use of facilities/equipment to the undersigned. It is agreed that the instructor shall not be responsible or liable to the undersigned for articles lost or stolen whilst making use of the facilities/equipment.

I acknowledge that I have read this document in its entirety, understand and agree to the above. I accept and undertake the risks. I accept that The Slink Academy is not liable for any accidents/incidents, illnesses or injuries that occur.

• I have been informed of the need for medical approval before participating in any classes/exercise programmes.

• I understand that any class undertaken with The Slink Academy, as with any other form of exercise, may result in injury if done incorrectly. I understand that all accidents will be documented and that I will be informed.

• If the instructor requires further information about the child’s illness or disability, it is my responsibility to make this information available and agree to update The Slink Academy with relevant information.

• I understand that the child may incur bruises due to the nature of the activity, and I consent to physical spotting occurring during the sessions for safety.  

• I understand that if the child fails to behave in a manner that is polite and social, he or she may be excluded from that particular activity at the discretion of the instructor.

• Pictures may be taken of the child during the class for social media purposes. It is your responsibility to inform the instructor if you do not want these photos or videos to be taken.

• I understand that personal information will be held and stored securely by The Slink Academy and give consent for this.
